The one-year pilot, announced last week, permits Legion Health's AI chatbot to renew prescriptions for certain psychiatric medications without requiring a physician to review the case. Utah-based patients pay $19 per month for what the San Francisco startup describes as "fast, simple refills." It is only the second time any U.S. state has delegated this kind of clinical authority to an AI system. The first was New Jersey, which approved a similar program last year.
State officials frame the pilot as pragmatic necessity. Utah faces a documented shortage of psychiatrists, particularly in rural counties where residents may drive hours for a 15-minute medication review. If an algorithm can handle routine prescription renewals, the logic goes, human doctors can focus on patients who need actual assessment. The cost savings—$19 monthly versus a $150 psychiatric visit—are presented as democratizing access.
Physicians are not convinced. The American Psychiatric Association and several Utah-based practitioners have raised concerns that the system's decision-making process remains opaque. When a patient requests a refill, the chatbot evaluates request frequency, reported symptoms, and pharmacy records. But what happens when that patient is also taking a medication the chatbot doesn't know about?
